        Please specify in the "Comments Box" exactly what type of appointment
        you desire.  
                  - New Patient
                  - Adjustment
                  - Massage - 1/2 hr, 1 hr, or 1 1/2 hr
                             If you are a first time visitor to our office, include "New Massage
                             Patient" in the comments box
                  - Both - Adjustment and Massage 
                  - Golf Program Participant
                  - Consultation

(During non-work hours, please leave your name, phone number and best time for us to return your call.) We check and respond to all messages at the beginning of each 'Administrative' Shift. We will call you to confirm the time/date based on availability.
